What’s the Difference Between PHP and IOP?

PHP and IOP are options for outpatient mental health programs, offering different levels of support.  PHP generally provides full-day treatment, five times a week. IOP typically provides treatment three days a week, in half-day (~3 hour) sessions.

Both can help when weekly or biweekly therapy is not enough, but inpatient care is not needed. 

These terms can also be used in substance use treatment, but here we are talking specifically about outpatient mental health treatment.

What is PHP?

PHP stands for Partial Hospitalization Program. The name can sound serious, and it is a higher level of care. But, PHP does not mean sleeping overnight in a clinical facility. Despite the name, there are no hospitals involved. You attend various treatment sessions during the day, then return home or to another supportive setting. 

A PHP may be recommended when mental health symptoms are making daily life feel harder to move through. This may look like mood changes becoming harder to manage, or trauma symptoms showing up in work, school, relationships, or self-care.

PHP can offer a steady rhythm during the day, giving someone full support while they begin to understand what is happening and what may help.

What Happens During Mental Health PHP Treatment?

A mental health PHP may include:

  • Group therapy
  • Individual therapy
  • Psychiatry or medication support when appropriate
  • Coping skills and emotional regulation tools
  • Support with routines, boundaries, and daily stability
  • Planning for the next step in care

 

PHP gives people a place to practice skills, understand symptoms, and begin feeling more confident before stepping into a lower level of support.

What Is IOP?

Participants engaging in group therapy during a PHP or IOP outpatient mental health treatment program.

IOP stands for Intensive Outpatient Program. It is still structured care, but it takes fewer hours each week than PHP. Most IOPs meet two or three days a week for a few hours per session.

IOP can be a good fit when someone would benefit from consistent support, but also needs treatment to fit around part-time work, school, family, or other daily responsibilities. It is often a natural step after PHP, when someone is ready for a little more flexibility while staying connected to care.

One helpful part of IOP is that mental health treatment can happen alongside everyday life. You can practice new skills during the week, notice what feels difficult, and bring those experiences back into treatment for support.

What Happens During Mental Health IOP Treatment?

A mental health IOP may include:

  • Group therapy and skill-building groups
  • Individual support
  • Psychiatry when appropriate
  • Help with stress, relationships, mood, and coping
  • Support while returning to daily responsibilities
  • Planning for ongoing care

IOP is about building consistency. It gives support without asking someone to step away from everything completely.

PHP vs. IOP: The Main Difference

The biggest difference between PHP and IOP is time and structure.

PHP is the more intensive option. It may fit when symptoms are difficult to stabilize, daily functioning has been disrupted, or someone needs a stronger treatment schedule during the day.

IOP is usually more flexible. It may fit when someone still needs regular support but can manage home, part-time work, school, or family responsibilities with help.

A simple way to compare them:

  • PHP: The most support hours and structure offered in an outpatient setting.
  • IOP: Fewer hours, more flexibility, continued support in daily life.
  • Both: Outpatient therapy, skills, and support without staying at a facility overnight

Which Outpatient Treatment Option is Right for Me?
